@manishkumar Your questions are in italics and my answers are in maroon color.
1. Is the hotel booking for processing visa to Kuala lumpur is mandatory?
Yes.
2. If I go to Kuala lumpur and after 2 days If I plan to go Phuket then how should I get Visa ?
Doesn’t change anything. Get the visa for Malaysia as usual. They just need your hotel booking and confirmed departure tickets.
3. Is there on arrival visa available in Phuket?
Yes.
4. What all places to visit in Kuala lumpur?
I don’t have any specific suggestions apart from the usual, popular places. http://www.kuala-lumpur.ws/attractions/10-thing...
5. We are going for 5 days so can I visit the good places in Kuala lumpur and Phuket both?
Yes. But it will feel hurried and you won’t be able to absorb (/process) the experience.

I need some more info If u have any Idea..
1. If I am plannning to go to Phuket then still hotel booking for Malaysia for 5 days required. As If I am going to Phuket and I am booking 5 days of stay in Kuala lumpur then maybe its waste of money..
Is there any solution for this?
2.Is there any way to just show temperory booking at the time of visa processing and I ll cancel booking once I got the Visa.after that I ll book hotel as per my convienience..

Just re-read your second post in detail and realized what the problem is.
In simple words, you can’t do what you are planning on doing. Once you leave Malaysia to visit Thailand (Phuket), you will need another visa for re-entry to Malaysia (Kuala Lumpur) AFAIK.
I am going to issue Malaysia Visa first and after spending 2 days in Kuala lumpur I am planning to go Phuket.
I can tell you my plan in detail so that It will be easier to help me..
1.I am travelling from Hyderabad to Kuala lumpur on 29th Sept Night.
2.I will reach on 30th morning and I am planning to stay till 2nd morning in Kuala lumpur.
3.On 2nd Oct in afternoon I am planning to go Phuket.
4.I ll be in Phuket from 2nd Oct to 4th Oct afternoon..
5.I will return to Kuala lumpur back on 4th evening and depart to back to HYB in night.
We friends are going(Not with family)
Could you please give me some more info so that we will plan easily.
I have 1 more que as Is it worth to stay 5 days in Malaysia instead of going to Phuket.
Please let me know If you have any Idea..
@manishkumar I came to realize your tricky situation by re-reading your second post. You should have made this clear from the beginning. Anyway, see my previous post. You can’t enter Malaysia from India, leave for Thailand, and then re-enter Malaysia on a single Visa AFAIK.
If you also want to visit Phuket in the same trip, you will have to change your flights to the following: HYD to KUL, KUL to HKT, and finally HKT to HYD. I am assuming you still haven’t obtained the visa for Malaysia.
As for whether it’s better to spend 5 days in Malaysia, that’s a subjective question and the answer depends on your goals from this Holiday. I would have personally preferred a location like Phuket to relax and a location like Kuala Lumpur to re-energize.
The point is that I have already booked ticket to HYB to Kuala lumpur and Kuala lumpur to HYB.
Now I am also plannig to go Phuket from Kuala lumpur and after spending 2 days in Phuket, come back to Kuala lumpur and then return to HYB.
My question is If I take visa for Malaysia now and then I will issue on arrival Visa to Phuket.
after that while returning from Phuket to Kuala lumpur do I again need to issue Malaysian Visa?

Yes, bro. As I have mentioned twice already, I am almost certain you will need another visa to re-enter Malaysia from Thailand. I am making this statement based on my personal experience. Only the Malaysian embassy can give you the most accurate and up to date information about this.
As for the visa on arrival at Phuket, Indian nationals (i.e. Indian passport holders) get that facility regardless of which country they are coming from.
Thanks for your help.
If you dont mind then could you please let me know any offer to book Hotel in Kuala lumpur.
also If you have Idea then tell me in which area I should book the hotel?
@manishkumar Book hotels from Goibibo when they raise the limits on Gocash utilization. Combine with Uco/Axis offers for unbelievable discounts. Alternatively, check Expedia for prices.
My hotel choices will probably not fit well for a budget trip with friends. But whatever hotel you end up getting, make sure it’s close to a LRT/monorail station and, since you will be visiting only for a couple of days, try to find a hotel close to the city center.
Contact @jasskochhar, @Bagpiper, or @ishandon for advice on Goibibo discounts. Now I will be offline for the rest of the day. Will try to answer any follow up questions tomorrow.
