yes that's one time, if u want to use ur 1 lab test (which is included into care plan) then u can avail this pack by paying extra 325 + slot charges

there are two care plans - 3 months and 6 months. both apparently offer the lab test benefit, however, are the benefits exactly the same?

Always bought 6 month plan so can't comment on 3 months plans Benifits.

In 6 months plan what lab package is offered either totally free or any nominal charges?

U can choose cbc or h1abc for free, and at nominal charges some packages are there, just like the one mentioned in this post.
