Both are ok. Easier to manage if you have linked them together. You will get this offer on both cards even if they are linked together. I have got it.
What I see is that this offer is available for the variants – Membership Rewards and Gold Card. If you have the Gold Credit Card variant, the offer may not be applicable.

You need to create new account for suppl card I believe, I did that way and could save in my Suppl card too

Got confirmation from AMEX customer care regarding
if a txn failed or refunded that will not be qualified for the cashback and we can try again doing the txn from the same card
and also all the add on cards where the offer is showing and collected separately by login to those account all are eligible for the offer
You will get as many times as many S letter you have in your name
Amex allows 2 add-ons free per card and you have 3 S in your name
