Select ‘Add Promo Code’ at the bottom
Vaccination Promo Codes will be applicable in the Uber app for all users, in 34 cities, and across all our Products
Add the promo code for a trip to the nearest Ministry of Health and Family Welfare Authorised vaccination centre in a Govt. or Private hospital and the return trip.
Navigate to the rides home screen and enter the pick-up/drop-off location for yourself or the person you are booking for.
Confirm your trip
The value of each free ride will be maximum Rs. 150 and a rider is entitled to maximum of two free rides, to and from the vaccination center
The final fare displayed before confirming the trip will be inclusive of the discount
From March 8, we will roll out these free rides in a staggered manner across 34 Indian cities. These include:

Offer didn’t work for me today morning 9am in Kolkata.
I had added the promo code yesterday in my Uber account and it was successfully added.
I just took a ride with my Dad to take him to the Govt. Vaccination centre for COVID vaccination. However no promo code was applied to the trip and I have been charged in full. Tried complaining to Uber support but got app message that usage of promotions is currently disabled in my Uber account, however they didn’t disclose anything more.
Seems like this offer is a scam.
I could easily travel by auto by paying Rs.60 vs. paid Rs.120 to Uber (after 30% Amazon Pay discount).

Also v can’t complain if v dont c upfront discount applied before ride itself… No point in asking after taking ride… Uber won’t entertain it anyday

Still, this success is giving pain in the back of some, and they saying we trust doctors but not the govt.!!!
Abe to vaccine bhi to doctor hi lga rahe hai, konsa govt ka Minister lagane ayega. 
Few of my family member who works in Govt hospital, many friends and doctors whom I personally know and other hospital staff all got vaccination and all are fine.

Yes, in my case it did. The particular hospital should be registered with uber for giving covid vaccination. So in case if one hospital does not work u can try others which are in your close vicinity
