If a bank issues a loan against a property to an individual that is in someone else name ....can it be reported and asked to be taken action against ?

I don't think this is something reported to the BANKING OMBUDSMAN.

You may need to report it to the police / court case against the Bank. This is a case of fraud.

BUT BANK SHOULD HAVE CROSS CHECKED THE STATUS OF LAND UNDER DISPUTE .

As per banking policies it is not possible to issue loan on property unless hypothecation is created to banks. Only Owner can sign hypothecation agreement.  

if it is done without ur consent. Just file Police complaint.

Yes, Bank does physically check the land/property. A surveyor comes and submits the report of the said property. And property is also mortgaged.
