1. I majorly use Mobikwik, Amazon Pay or Freecharge for recharge, gas booking, electricity or water payment.
Earlier there was a issue in which I used wallet balance and fk axis cc for payment and cashback was not issued and at that time I used Freecharge as platform. At that time CC customer care replied that for transactions in which card as well as wallet balance is used, it is treated as Cash loading in wallet and cashback is not issued.
Today I was analysing my credit card spends and found for few transaction cashback is issued and for few it is not issued.
What should I do now? If I report them, can they reverse the cashback issued?
2. Purchased 2 wheeler policy from PB and used FK axis card. Cashback not issued. Is it standard or I have to report them for cashback ?
1.It depends on the payment gateway and also if you are using wallet balance + credit card it won't fetch cashback for most of the time. Wallet Loading is 0% in FLipkart AXIS Card.

Where they give cb ? Only fk?

Jis transaction ka cashback nahi aaya uske liye complaint karu kya mentioning other transactions for which cashback was issued.
Aisa to nahi piche wala cashback bhi reverse kar de.
Check merchant names on which you have received cb and tranacting on same merchant names will receive Cashback
Which transactions/categories in bill payments are you still receiving cashback?
Axis removed utilities from cashback long ago for fk card.
