After request,
you need to call customer care and mention SR number to get RuPay platinum debit card,
Note - Customer support also said, you will get same card variant what you have currently.
But most of the time they issue by default visa card. So, you need to mentioned you want RuPay platinum debit card
Already 5 Add-on RuPay Platinum debit card successfully Applied, Dnt know how many Add-on RuPay Platinum debit card you can apply. One more thing, one card they issue RuPay Classic card. so, you need told them you want only RuPay Platinum debit card.

Warning – Please dnt issue add-on card, some ICICI bank employees mistakenly re-issued your primary card and all your add-on card getting blocked
or apply for only one add-on card and before activate your card, please check your card is add-on or primary card.
It can be easily checked with the help of SR number – “ATM or debit card Reissue Request” for primary card reissue

Yes but they will issue any add-on cards. Most of the time they will issue Visa card. 
After online request submit, you will get SR number.

Tried my luck too since I have an ICICI Salary Account. Call customer care with the SR number. He told that he cannot modify the request so he just mentioned in the notes of the ticket that the customer has requested for basic Rupay Platinum Debit Card. Let’s see what they give me.

You have given request yesterday ?. Check in imobile app what card they issued, for few people they issued visa card only.

Crediting money from other bank account does not count as salary account. otherwise one would open salary account in every bank.
generally salary credit by company is a cms transaction not imps or neft.
