Visit any sbi branch in your locality, meet the sbi card agent there. The agent will help you apply for any sbi cc available against FD. Make sure to insist multiple times that you need only simplyclick card else you will cancel if any other card received as agents will try their best to give you irctc or simplysave card. Take PAN, a passport size photo & an address proof(aadhaar,DL,voter id). Min FD required is 20k & card limit will be 70% of FD amount(so 14k card limit for 20k FD, I suggest a FD of 45k so you get at least a 30k card limit). Keep in mind that card against FD once issued cannot be changed meaning limit will forever remains the same & card will also remain the same(no request for converting card to another type).
