I want to order a item which is available only on amazon.com costing 19$
I have a Amazon prime account.
How do I order?
How to pay in USD ?
Will there be any customs duty on it ?
How will the exchange rate be calculated ?

Amazon.com USA now also accept RuPay-Discover International cards, activate international E-com transaction before payment -

Yeah ordered a lot of items from there. Better for stuff which you don’t get in India. 
Indian prime account won’t be of any use to purchase on Amazon.com
First check if the product can be shipped to India and then go ahead with order just like on Amazon.in, no difference in process
Using any card that has international transaction enabled if paying in USD. Else, other option is to pay in INR using any card. The USD to INR price will be shown on checkout page. That would be the final amount billed.
Mostly yes. The shipping and customs charges are estimated and shown while at checkout. You have to pre pay all amount including product cost, shipping and customs before hand. If the customs duty is less eventually, you will get a refund. On most occasions, it would not cross beyond what Amazon bills but might go lower.
Indian Amazon Prime wont work on .com and you will have to pay customs as well. You can use your debit or credit card to order in USD if its enabled.
When buying items on Amazon.com you can see list of items which are available to be shipped to India along with custom and shipping charges.
