Procedure to close DCB Elite account

Please provide me the steps to close dcb account.
I am in tier 2 city need to go tier 1 city to close it. So don’t want to miss any documents.

They were ready to send their executive to collect documents even for closure. Try
I argued with CC, if they can come for a/c opening why not for closing and agreed to send but I decided to keep a/c for some more time

This closure is on my to-do list. This is what I included for now
I believe we should be able to get other necessary forms from bank/branch.
