To check the extension of the Jio Primemembership on your Jio account, go to the MyJio app and then visit the My planssection. The section will show you a notice confirming the extension of the membership for another year.

Yes. Visit the Jio store and ask for it. They will require you to submit all documents (ID and address proof) in hard copy. The sim will take some time to activate.
I got one sim that way for my father.
