The Free Dental Check-up Offer (the “Offer”) is conducted by Colgate-Palmolive (India) Limited (“Colgate”) in association with India Dental Association (“IDA. Offer is available in select cities / towns only as per the list below. Free dental check-up is a limited period Offer and is valid from 19th January, 2019 until 27th January , 2019 (“Offer Period”) . The Offer Period may be changed by Colgate at its discretion, without any notification. Any Shoppers who buys any value of Colgate products on Goffers website or app in select cities as mentioned above during the Offer Period can avail the free Dental Check-up. Shoppers will receive a link of Free Dental Check-up card from Grofers on their mobile number used for the purchase of colgate product between 19th January- 27th January 2019 i.e. (“Offer Period”) Please print the Free Dental Check-up Card from the link sent by Grofers. Dental check-ups can be availed from 19th January 2019 – 28th February 2019. No appointment scheduling will be allowed after 28th February 2019 and No free dental check-up can be availed under this Offer after 28th February 2019. Shopper can avail free dental check up on presenting the Printed Check-up card to the concerned clinic. To avail the free dental check-up: Give a missed call on 1800 267 6700. You will then receive a call back where the IVR (Interactive Voice Response) will direct you to further steps after which you will be prompted to enter your Area Pin code. Depending on whether you are calling from a mobile phone or a Landline Phone you will receive contact details of participating dentists as follows. Mobile Users: If you are giving a missed call from a mobile phone, you will receive an SMS with details of at least 2 participating dentists in or around the pin code provided by you, if available. If there is no dentist offering the free check-ups in or around your pin code area then you will receive a regret SMS. Landline Users: As the details of the dentists are shared via sms, one needs to call from a mobile phone to get the details for the free checkup. Call charges may apply as per your subscribed plan. Please coordinate with the dentist for an appointment as per the options provided, if any, to avail the free dental check-up. Colgate is not responsible for getting your appointment with the Dentists. Carrying the Free Dental Check-up Card received by the Shopper under this Offer is a must to avail the same and failing to produce the same shall not entitle the Shopper to avail the free dental check-up facility.Free Dental check-up includes only the routine check-up & scree Personal details such as your mobile/landline number(s) provided by giving a missed call and further details such as age, gender, alternate contact numbers, if any provided by the caller while following the IVR steps, will be stored by Colgate / its authorised agencies in a reasonably secured manner and the same shall be used only in relation to this Offer and will not be used or shared for any other purposes. For any assistance / clarification a toll-free number – 1800225599 will be available to you in case of any queries or concerns. The number will be available from 9:00am-6:00pm, Monday to Friday, between 19th January, 2019 and 28th February, 2019 excluding public holidays. Please note that Colgate is facilitating in providing a free dental check-up only, provided a dentist participating in this Offer is available in the vicinity of your pin code area. Colgate assumes no responsibility for the services of the dentists during free dental check-up. Colgate shall not be liable for any cost or consequences for any additional treatment / procedure availed by any person on his / her own volition or on advice of the Dentists. Colgate does not subscribe to any opinion / suggestion given by the participating Dentists in their independent professional capacity. Colgate is not liable for any lack of or deficiency in service provided by the Dentists. Colgate reserves the right to extend, curtail or alter these terms and conditions or cancel this Offer in its sole discretion. Disputes if any will be subject to the Jurisdiction of the Courts in the city of Mumbai alone. Free dental check-up is intended for generating Oral Care awareness and is not intended for any commercial gains. The holding of the free dental check-ups is subject to force majeure conditions.
