you can also apply through HDFC branch but branch executive should apply the Code: RRR01 in ATM/Debit card forum and also told them wrote a mail to head office Specifying the card name (i.e RUPAY PREMIUM DEBIT CARD).
Nice I got this card when I opened my account few years back ,swapped it for Millennia 
Usually rural areas have this kit
Usually all banks give rupay variant except for that pathetic Axis .
They are such thieves even a normal debit card cost 360 pa
That’s why I opened Bsbda Account with them
Finally got a RUPAY PLATINUM from HDFC Bank using the Card Code: RRR01 by filling the form and applying through the Branch Customer Care Rep. with all the great research and help from @equalizer and our Rupay Specialist @abhishek012

Most of the people get HDFC RuPay Platinum debit card with Code: RRR01
Previously they use to have option for rupay platinum via netbanking along with reward card, jet airline card n times card.
Later removed most of these but added business n millenia.
Hdfc is on the verge of becoming worst than public sector bank in terms of netbanking and mobile banking, other banks are adding features and hdfc is removing whatever is left making customers life hell and forcing them to visit branch every now and then just like PSU banks
Now HDFC RuPay Platinum Contactless variant also available -
Filled e-age form from this post and chose other card with code RRR01.
Submitted in the nearby HDFC Bank branch last Saturday and within 2 working days I can see this in my banking app 
Very fast service and special thanks to @abhishek012 for motivating me to apply this card with his constant RuPay posts 
Thise hesitating to apply as we need to go to branch please don’t waste your time writing e-mails simply fill and drop it it’s a few minute process.
Will update the charges which got debited shortly as and when theg get debited
Card got dispatched yesterday from Mumbai; I was very skeptical if it would arrive by today (today being the last Friday of August would have saved me extra ₹100) and fortunately it did arrive on time today and i instantly used it on Amazon worked flawlessly as expected 
Those who are yet to apply follow what @abhishek012 @equalizer has mentioned blindly don’t waste your time mailing them just fill a simple form and drop at nearby HDFC ATM/branch it’ll be done automatically without any fuss or hassle

Did you fill manually by hand ? or did you fill it using some PDF tools ?

@abhishek012 i have easy shop platinum with bin number 652166 but it is not working on amazon pay offer other cards are working fine.
Offer working only once per month. remove the card and add again.
This cards comes as default for hdfc farmers account with 2500 MAB. I have it…. No fuss… The account can be opened online… Normal vkyc process… Nothing to prove that you are a farmer…. 🙃
yes , a family member have same account , but it’s debit card is not working for this offer 🙃 don’t know what’s the problem.
For a newbie like me, can someone explain what means “drop in ATM?” Do ATMs have dropboxes also? I’ve only seen card machine in my HDFC ATM.

NOTE: Its a 5 digit code [RFL1 : is not listed in the computer .. invalid]
RRR01 : may be some monthly charge
RRFL1 : Free Card – No charges

Did you see any other RuPay Platinum debit card codes or any other RuPay card codes ? because HDFC RuPay Select debit card also available but dnt know which code to apply.

Since I have got my Existing Credit card upgraded to HDFC Millennia Credit Card now.
So planning to save yearly fees by downgrading my Millenia Debit Card to Rupay Premium .
Will most probably use the RRFL1 code.
What about old debit vard?
I head hdfc allows only 1 debitcard
